  • Accessing the MLR Report and filing instructions, which are available in the Health Plan Management System (HPMS) under HPMS Home > Plan Bids > Medical Loss Ratio > CY2024.
  • Uploading completed CY 2024 MLR Reports to HPMS. The functionality for this will be available from November 7, 2025.
  • Submitting these reports by 11:59 PM PST on December 5, 2025.
  • Completing and submitting attestations for CY 2024 MLR Reports in HPMS starting from December 8, 2025, and no later than 11:59 PM PST on December 17, 2025.

2. Ensure the submission of completed CY 2024 MLR Reports in the HPMS between Friday, November 7, 2025, and by 11:59 PM PST on Friday, December 5, 2025.

3. Use the attestation functionality in the HPMS, available from Monday, December 8, 2025, to submit CY 2024 MLR attestations by 11:59 PM PST on Wednesday, December 17, 2025.
